Please be advised that the Administrator is NIKAMED Centrum Zdrowia Monika Waligóra based in Warsaw, 6/9 Nowowiejska Street, email: registracja@nikamed.pl. The contact details of the Data Protection Officer are the same as the contact details of the Administrator indicated in the previous sentence.

Personal data will be processed for the purposes of correspondence and in accordance with the content of the consents granted to the Administrator. The legal basis for processing is the consent of the data subject.

The data will be processed until the purpose for which they were collected ceases to exist or until the objection to the processing of the personal data of the data subject ceases to exist, whichever occurs first. Everyone has the right to request the Administrator to access personal data, rectify it, delete it, limit processing, as well as the right to transfer data and object to processing. Everyone has the right to withdraw consent to the processing of personal data that was given to the Administrator. Withdrawal of consent does not affect the lawfulness of the processing that was carried out before its withdrawal. Everyone has the right to lodge a complaint with the Supervisory Authority [after May 25, 2018: to the President of the Personal Data Protection Office].

The rights of the data subject, including the withdrawal of the consent granted, may be exercised by submitting a statement using the contact details of the Administrator or the Data Protection Officer indicated in the introduction.
